A valid prescription or doctor’s order

Your insurance card or policy number

Your diagnosis or condition

Any recent clinical notes (if available)

What Insurance Typically Does Not Cover:

Lift Chairs (may only cover the lift mechanism portion)

Scooters & Power Chairs (unless strict criteria are met)

Ramps & Grab Bars

Bath Safety Equipment (most items)

Incontinence Supplies (some plans may cover)
