What Is a Waiver Program and What Does It Cover?

A Medicaid Home and Community-Based Services (HCBS) Waiver helps eligible Minnesotans receive care and support in their own homes or communities rather than in a nursing facility or institutional setting. How to Get a Waiver in Minnesota
Step 1: Determine Eligibility
Waivers are available to people who:
- Are eligible for Medical Assistance (Minnesota Medicaid)
- Require a nursing home or institutional level of care
- Prefer to live in their own home or community setting
Step 2: Contact Your County or Tribal Human Services Office
Each Minnesota county and tribal nation has a lead agency that handles waiver applications. Contact your local county’s Aging & Disability Services office to request a MnCHOICES assessment — this determines medical and functional eligibility.
Step 3: Complete the MnCHOICES Assessment

Step 4: Work with a Case Manager
