What Is a Waiver Program and What Does It Cover?

Helping Minnesotans Live Safely and Independently at Home

A Medicaid Home and Community-Based Services (HCBS) Waiver helps eligible Minnesotans receive care and support in their own homes or communities rather than in a nursing facility or institutional setting. Waivers are designed to promote independence, dignity, and quality of life — allowing individuals to live safely at home with the services and equipment they need.

What a Waiver Can Cover

Each waiver type (CADI, DD, BI, EW, etc.) offers coverage for a wide range of services and equipment based on individual needs. Examples include:

Personal and Home Care
  • Personal Care Assistance (PCA)
  • Homemaking, meal prep, and cleaning help
  • Respite care for family caregivers
  • Companionship or supported living services
Medical Equipment & Supplies

How to Get a Waiver in Minnesota

Step 1: Determine Eligibility
Waivers are available to people who:
  • Are eligible for Medical Assistance (Minnesota Medicaid)
  • Require a nursing home or institutional level of care
  • Prefer to live in their own home or community setting
Step 2: Contact Your County or Tribal Human Services Office
Each Minnesota county and tribal nation has a lead agency that handles waiver applications. Contact your local county’s Aging & Disability Services office to request a MnCHOICES assessment — this determines medical and functional eligibility.

Step 3: Complete the MnCHOICES Assessment
A certified assessor will visit your home (or meet virtually) to evaluate your daily care needs, home setup, and support system. This assessment helps determine which waiver program best fits your situation — such as CADI, Elderly Waiver (EW), Brain Injury (BI), or Developmental Disabilities (DD).

Step 4: Work with a Case Manager
Once approved, a case manager is assigned to help create your personalized care plan and coordinate services, equipment, and supplies — often through approved vendors like Dahl Medical Supply.

how it works

  • Get a Prescription or Authorization

    Ask your doctor, nurse practitioner, or case manager to write a prescription or authorization for the supplies or equipment you need.

    The prescription should include:
    • Your name and date of birth
    • Diagnosis code (ICD-10) related to your condition
    • The type of product (for example: incontinence supplies, wheelchair, lift chair, etc.)
    • Quantity and frequency (how often you use the product)
    • Length of need (for example: ongoing, 6 months, 12 months)
    If you’re on a Minnesota waiver program (such as CADI, EW, BI, or DD), ask your case manager to send your service authorization directly to us.

  • Send Your Documents to Dahl Medical Supply

    Once you have your prescription or authorization, send it to us:
